Nose job
Rhinoplasty (which is commonly referred to as a nose job) is a procedure in which the nose is re-sculpted using various techniques better facial balance.
Nasal plastic surgery is the most artistic and most complex of all facial plastic surgery procedures. Within the plastic surgery specialties, Rhinoplasty is considered the queen of the surgeries, due to its high level of complexity.
Being one of the most popular procedures in cosmetic surgery, it can be a life changing experience in a person’s life. We all know the importance of having a harmonious and nice face and we focus on proportional and aesthetic balance to the face when evaluating a patient for nose surgery. We meet with each patient individually to discuss the desired outcome as well as discuss realistic expectations given a patient’s unique nasal structure.
INTRODUCTION
Because it is a crucial element in the pursuit of well-being, success and harmony with oneself, beauty has become a requirement of all. For a long time it was considered as a gift from heaven but thanks to cosmetic surgery almost anyone can achieve the beauty we want today. There are of course the financial constraints and physical limitations but apart from that we can make your dream come true.
This is an important decision that should never be taken lightly as its effects are important, physically but also psychologically. The decision should not be motivated by a third party to obtain secondary benefits (“I do it to please someone”) but for oneself.
Rhinoplasty is probably the most delicate cosmetic procedure as the nose is the central feature of the face. It is a very common operation (between twelve and fifteen thousand operations a year in France). Its purpose is to correct an imperfect nose on the aesthetic plan of course but also to improve respiration problems. The difficulties in respiration could be one of the reasons a patient come to see his Doctor.
The preoperative consultations are essential for this type of intervention as a perfect agreement is essential between the patient and the surgeon. This needs to be done before the surgery. This evaluation needs to focus not only on the defects to be corrected but also from a psychological point of view.
The surgeon should have preoperative photographs (face, profile, three-quarters) at rest and with a smiling face. These photos allows a precise analysis of defects to be corrected (change the proportions, the shape..) The photos will then be compared with postoperative photos to see the improvement. This is a great tool that will be the base for the co-operation between the patient and his Doctor from the very beginning. New technologies such as computers are a valuable addition to the photographic analysis of the future nose. Indeed, at the request of the patient, a computer simulation can be performed to better visualize the corrections that will be made.
WHAT CAN BE DONE?
The nose has the distinction of being a complex structure in three dimensions “based on the void” and maintained by its bony attachments; its laxity is related to its components and cartilaginous tissue. The skeleton of the nose can be considered as a roof, supported by a partition. The sides include a portion of the roof top bone, formed by the bones and ascending branches of the premaxillary and a cartilaginous portion at the bottom. The nasal septum is composed of two parts, bone and cartilage. The fleshy external end of the nasal septum is sometimes also called columella.
Thus, the nose is composed of many complex anatomical elements each of which may be the seat of deformation, alteration or functional unsightly, and may be subject to a specific surgical procedure (bone, cartilage, mucous , wall or skin). Most cases are reduction rhinoplasty, with shortening of the nose and lowering the profile line. The procedure is usually performed under general anesthesia providing greater comfort for the patient and the surgeon.
Reduction Rhinoplasty reduces the size of the framework of the nose over which the skin is draped. The skin itself is not touched. The frame of the nose which is made up of bone in its upper half and gristle (cartilage) in its lower half is approached from underneath the skin trough cuts which are made inside the nostrils. This is achieved by a hidden path, called endonasal where the scars are hidden inside the nostrils. In difficult cases (advanced surgery, deviated nose, secondary rhinoplasty), there might be a small external scar.
Once the modeling of the tip is obtained and after possible correction of the nasal septum, the intervention will focus on the correction of the nose bone. Techniques often include cartilaginous excision and bony hump reduction with a chisel or a rasp. The intervention ends with the closure of incisions. The nasal structures are carefully and meticulously compressed to reduce the swelling and special tapes are placed on the surface of the nose along with a custom-molded nasal splint to further improve the shaping and reduce the swelling of the nose which will occur after surgery.

Facial surgery
I recently had a large benign bone growth removed from behind my ear and at the time jokingly suggested that they could give me a lift at the same time (they being; Dr Braccini).
Well, the joke became reality and although I was somewhat anxious that I might look artificially enhanced, (and sadly we see some women who have taken on that ‘overworked’ look) this wonderful doctor assured me that I would look very natural.
It is now several weeks since I had these two procedures and the result is that I no longer have the sore neck from the bone growth but have also a nice surprise when I look in the mirror each morning.
The face reflecting back looks like it has been on holiday, totally natural with the saggy chin a thing of the past. My friends have commented that I’m looking really well, some even say ‘great”.
